Bank Direct Utility Payments

To have your utility bill amount withdrawn directly from your bank or credit union monthly, complete the authorization agreement and mail or deliver it to the Centerville City Offices, 250 North Main Centerville, Utah 84014. Don't forget to include a voided check.
