Registration

To Register O-Line, Go to “www.activityreg.com” 

Starting March 1st and ending April 6th -

 • Step 1: Enter UT for the State or click on State

 • Step 2: Select your facility  - Centerville City Recreation

 • Select  - Programs under MENU

        • Click on the arrow for Make selection here and 
        • choose: Baseball/Softball/T-Ball       
        • Then click on the Registration button and 
           fill out the Registration Information and Submit

                 Note: Birth Certificates will be required at City Hall

Starting March 12th and ending April 6th -

 • Forms, Fees and Birth Certificates will be received at City Hall  250 N. Main - Centerville, UT
